Innovative Provider Solutions

At the heart of health care is the physician-patient relationship. Our Integrated Health Management Solutions (IHMS) initiative will support and strengthen your relationship with your doctor by providing tools and resources to promote efficient, effective and coordinated patient care. The system rewards providers for meeting quality care standards and delivering effective health outcomes.

The Patient-Centered Medical Home (PCMH) initiative, which is one component of IHMS, aims to improve coordination of care for members and strengthen management of chronic conditions, such as diabetes and asthma. PCMH will focus on each member and the personal physician of their choice, coordinated patient care, quality and safety, enhanced access to services, and appropriate payment for providers.

Health care today has become fragmented among various specialists and programs. The idea for medical homes came from a desire by physicians to make medical care more comprehensive, as it was in the past when a family doctor managed all of your care. HMSA is currently piloting a variety of PCMH models with different provider partners to test alternative delivery methods and financing systems.
